Advice on Health insurance company who wont pay!?

I rang my health insurance company as I had sore back and was given authorisation to have chiropractor work carried out. Was given authorisation code etc. Have now had �300 worth and insurance company now say want doctors report. I dont have one as they told me at start that i didnt need one..... They are just doing anything not to pay.. any advice?
